Description
Responsible to provide person-centered services to individuals with disabilities primarily focusing on developing social and vocational skills in a facility-based setting.
Tasks
- Provide person-centered support across Work Center, Blaisdell, and In-Home programs.
- Follow ISPs to deliver proper supervision and assistance during all service times.
- Ensure participant health, safety, and support with activities of daily living.
- Promote community integration, employment, and meaningful participation.
- Accurately complete documentation and report incidents per regulations.
- Keep program areas clean, organized, and safe for all individuals.
- Uphold agency policies and model respectful, professional behavior at all times.
Qualifications / Education
- High school diploma or GED; preferred a minimum of one years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
- Ability to read and understand Individual Support Plans and schedules.
- Demonstrate written, verbal and organizational skills.
- Maintain a professional attitude and appearance.
- Strong decision-making skills. Must be dependable and flexible.
- Ability to work in a team environment with all levels of the organizations.
- Ability to apply basic first aid skills.
- Must have reliable transportation.
