Description

Responsible to provide person-centered services to individuals with disabilities primarily focusing on developing social and vocational skills in a facility-based setting.

Tasks

  • Provide person-centered support across Work Center, Blaisdell, and In-Home programs.
  • Follow ISPs to deliver proper supervision and assistance during all service times.
  • Ensure participant health, safety, and support with activities of daily living.
  • Promote community integration, employment, and meaningful participation.
  • Accurately complete documentation and report incidents per regulations.
  • Keep program areas clean, organized, and safe for all individuals.
  • Uphold agency policies and model respectful, professional behavior at all times.

Qualifications / Education

  • High school diploma or GED; preferred a minimum of one years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
  • Ability to read and understand Individual Support Plans and schedules.
  • Demonstrate written, verbal and organizational skills.
  • Maintain a professional attitude and appearance.
  • Strong decision-making skills. Must be dependable and flexible.
  • Ability to work in a team environment with all levels of the organizations.
  • Ability to apply basic first aid skills.
  • Must have reliable transportation.
