Community Participation Support Coordinator

Description

Responsible to coordinate person-centered services in the community through volunteerism and other engagement opportunities that may lead to potential employment.

Tasks

  • Manage service authorizations and monitor utilization to ensure ISP alignment.
  • Supervise staff and coordinate schedules for efficiency across employment programs.
  • Support billing, payroll, and documentation to ensure service effectiveness.
  • Build and maintain community business partnerships for employment opportunities.
  • Market services to businesses and promote inclusive community-based employment.
  • Ensure participant health, safety, and person-centered support per ISP.
  • Provide staff training, quality control oversight, and program area organization.

Qualifications / Education

  • High school diploma or GED; preferred a minimum of five years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
  • Support program management by ensuring the provision of high quality supports and services.
  • Ability to read and understand Individual Support Plans and schedules.
  • Demonstrate excellent written, verbal, and organizational skills.
  • Maintain a professional attitude and appearance.
  • Strong and independent decision-making skills.
  • Must be dependable and flexible. Ability to work in a team environment with all levels of the organization.
  • Ability to apply basic first aid skills.
  • Must have reliable transportation
