Social Worker

Participates in Meetings and Care Plan Reviews providing information on individual resident's psychosocial condition, assists resident and families to maintain highest psychosocial quality of life possible.

• Conducting comprehensive assessments of residents' physical, emotional and social needs.
• Providing counseling and support to residents and their families.
• Facilitating communication between residents, families and staff.
• Coordinating resources and services that can assist residents and their families.
• Developing programs and activities that promote resident engagement and well-being.

• Minimum of Bachelor’s degree in related human services field including but not limited to sociology, psychology, special education or rehabilitation counseling.
• Position requires two years’ experience in a health care setting working directly with individuals, preferably with at least one-year experience with long-term care.
