Description

Classify residents into levels of care for reimbursement purposes. The incumbent coordinates all disciplines to complete the tasks within the regulatory time frame.To assure completion of the State/Federal required MDS within the appropriate time frames to organize and conduct the Resident Care Conferences. To assist the individual nursing units in the use of the MDS through group and individual teaching and training. To assure the MDS is complete and accurate.

Tasks

  • Chair Resident Care Conferences with team, residents, and families to review and plan care.
  • Ensure timely completion of MDS assessments for admissions, changes, quarterly, and annual reviews.
  • Develop and update resident-centered interdisciplinary care plans based on MDS and team input.
  • Engage residents and families in care conferences; address their concerns and follow up.
  • Supervise nursing units; monitor and report changes in resident conditions promptly.
  • Review MDS, care plans, and records for accuracy; update as residents’ conditions change.
  • Educate nursing staff on documentation standards to support accurate MDS and care planning.

Qualifications / Education

  • Current Pennsylvania RN license
  • Knowledge of State/Federal regulations
  • Minimum of one year nursing home experience as staff/charge nurse and knowledge of the MDS system
