Wellness Coordinator (LPN)

Description

Perform nursing practices and routines in the absence of a registered nurse (RN) and under the supervision of a physician, Director of Nursing Services, or RN, in accordance with federal, state, and local regulations and guidelines. A LPN assists in the supervision of Nursing Assistants and promotes a positive physical and psychosocial environment amongst residents and staff.

Tasks

  • Observes residents, charts and reports condition changes, and acts per facility policies.
  • Administers medications and treatments under licensed supervision; records times and dosages.
  • Conducts resident rounds, monitors care, and promptly responds to residents’ calls.
  • Supervises Direct Care Staff, providing orientation, training, and ongoing support.
  • Delivers basic resident care: vital signs, wound care, catheterization, hygiene, and mobility assistance.
  • Reviews MDS, care plans, and documentation for accuracy as assigned.
  • Collaborates with healthcare team to assess needs, plan care, and support Direct Care Staff.

Qualifications / Education

  • High school diploma or its equivalent is required.
  • Current LPN license is required, and the employee must be in good standing with the Commonwealth of Pennsylvania. Possession of a current Basic Life Support for Health Care Providers CPR certificate or enrollment in a CPR class may also be necessary.
  • Sufficient education in order to perform daily tasks with an understanding of the duties involved.
