LPN Charge Nurse - Skilled Care

Oversees the general Nursing care of all residents in the facility. Primary professional nursing care provider on the unit, completes medication administration, treatments, and oversees personal care of residents. Monitors residents care plans and delivers individualized resident care.

• Supports health director to manage daily healthcare operations and meet residents’ needs.
• Ensures dignity and quality care as residents’ healthcare needs change.
• Assists management with resident retention efforts.
• Partners with management to execute marketing plans and reach occupancy goals.
• Maintains or improves occupancy and revenue per marketing and business plans.
• Monitors residents’ health and well-being to identify move-out risks; implements solutions.
• Explains benefits of community care to prospective residents and their families.

• Graduate of an approved course in Licensed Practical Nursing with a current license to practice nursing in Pennsylvania. Previous experience as an LPN in an extended care facility preferred.
• Must obtain Pre-Hire Physical
• Must obtain 2 step TB
• Registered Apprenticeship program available.
