Service Coordinator

Responsible for providing case management services and coordinating care with other service delivery systems by performing the duties outlined in this job description.

• Reporting to work in a timely manner when scheduled.
• Participating in team meetings when requested to do so. Communicating necessary information to other employees and key stakeholders.
• Attending and completing scheduled required annual training.
• Functioning as a liaison between individuals, facility, families, the placing agency and the community.

• Bachelor's degree in social work, psychology or another human services related field. Experience in the behavioral health field, with at least one year of experience in a mental health setting, one year in an ID setting, and preferably one year of experience in the provision of case management services is preferred but not required. Required to complete the Blended Case Management training within one month of hire.
