Description

Responsible for assisting the Director of Integrated Outpatient Services with the oversight of clinical and management components of the Blended Case Management program in assisting severely mentally ill individuals to be able to live successfully in the community by linking/referring to services needed, monitoring service delivery to determine progress, gaining access to services, assessment and service planning, problem resolution, informal support network building, wellness coaching, utilizing community resources, and crisis response. This position is also responsible for the overall coordination of care and resources for consumers within the Outpatient Clinic. The Program Manager of Integrated Outpatient Services oversees the management of patient registries and oversees the outreach and engagement activities completed by the Outpatient Clinic reception department. This position oversees and ensures that each consumer has a smooth and professional experience with the agency, and is directed to the most appropriate level of care and resources.

Tasks

  • Functioning as a liaison between individuals, facility, families, the placing agency and the community
  • Attending and completing required annual training
  • Ensuring documentation and paperwork is completed correctly, signed, and filed in a timely manner
  • Direct supervision and management of the daily functions Multi-task between identifying areas for improving patient relations, ensuring that quality standards are met while providing extensive personalized customer service

Qualifications / Education

  • Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education, or be a registered nurse.
  • 5 years mental health direct care experience, two (2) of which shall include supervisory experience; or
  • Bachelor's degree in nursing and three (3) years mental health direct care experience. Experience in managing population health preferred.
